Section 174(2)(c) An Approved Provider must notify the Regulatory Authority of information in respect of any other prescribed matters. 

Regulation 175(2) provides a list of matters that are prescribed. This list includes any change to the ages of children being educated and cared for by a centre-based service.

If there is a change to both the nature of care offered and the ages of children being cared for, then you need to complete two separate SA12 forms to notify of both changes.
